— document for registered activity (DRD form) as proof that the bidder is registered as a natural person or a legal entity for conducting the activity related with the subject of the public procurement contract or proof that the bidder belongs to the relevant professional association in accordance with the regulations of the country where the bidder is registered,

— statement of the economic operator that no effective verdict has been issued against the economic operator within the past 5 years for a committed crime pursuant to Article 88 paragraph 1 of the LPP,

— confirmation stating that no bankruptcy procedure has been initiated by a competent body,

— confirmation stating that no liquidation procedure has been initiated by a competent body,

— certificate of paid taxes, benefits and other public duties issued by a competent body in the country where the economic operator is registered,

— confirmation from the Register of penalties for committed criminal offenses of legal entities proving that no secondary penalty —…

— list of main deliveries of goods or services provided with the values, dates, purchasers (contracting authorities or economic operators), with a certificate for completed deliveries or services provided: Certificates for minimum 3 implemented contracts for the same or similar equipment as the subject of the procurement. The certificates should be issued by the service users with specified values and dates of implemented contracts,

— the educational and professional qualifications of the service provider or contractor or their managerial staff, provided they are not evaluated as criterion for selection for the most favourable bid:

• statement that the person is employed with the economic operator, review of registrations/deregistration for mandatory social insurance for the business entity issued by the Employment Agency of the Republic of North Macedonia or the relevant contract for the hired person and their availability for the implementation of the contract, specifically minimum…

— certificates and reports issued by the bodies competent for quality control with recognized competences, and who conduct testing of suitability of products as was clearly determined with the references to the specifications and standards:

• original brochures from the manufacturer of the offered equipment with exact specifications and clearly highlighted offered characteristics of the products to be delivered.

• authorization from the manufacturer (MAF Manufacturer Authorization Form) for the procurement in question, which the contracting authority shall additionally check with the equipment manufacturer

— quality systems standards:

• Certificate ISO 9001:2015 Quality Management System or equivalent;

• Certificate ISO 20000-1:2011 IT Service Management System or equivalent;

• Certificate ISO 27000-1:2013 Information Security Management System or equivalent.
