--Certificate of registered activity (form DRD) as a proof that it is registered as a physical or legal entity to perform the activity related to the subject of the public procurement contract or a proof that it belongs to an appropriate professional association in accordance with the regulations of the country where it is registered. The competent authority in the Republic of North Macedonia for issuing this document is the Central Register of the Republic of North Macedonia.

- A decision confirming that the Economic Operator fulfills the requirements in terms of space, equipment and personnel for the activity of wholesale trade in medical supplies and is approved for wholesale trade in medical supplies, issued by the Ministry of Health of the Republic of North Macedonia.

--- A statement by the economic operator that in the last five years it has not been rendered a verdict for any criminal act as stated in Article 88 item 1 from the PPL

- Confirmation that there are no bankruptcy proceedings opened against the operator by a competent authority

- Confirmation that there are no liquidation proceedings opened against the operator by a competent authority

- Confirmation for paid taxes, benefits and other public duties from the competent authority in the country where the Economic Operator is registered.

- Certificate from the Registry of penalties for criminal acts by legal entities that minor penalty…

… ban on participation in public tender procedures, award contracts and contracts for public – private partnership has not been imposed

… has not been imposed temporary or permanent ban for performing separate activity

- Confirmation that a final judgment for sanction a prohibition of professional activity or duty i.e. temporary prohibition for performing separate activity has not been imposed.

Техничка и професионална способност:

- with a description of the technical personnel and technical bodies that will participate in the execution of the public procurement, especially those who are responsible for quality control, and in the case of public procurement of works, those who will be involved in the performance of the works, regardless on whether they are employed by the economic operator or not:

-in the case when complex goods or services are procured, or in exceptional cases for special purposes by checking the manufacturing facilities, or if necessary also the facilities for study and research, as well as the quality control measures, carried out by the contracting party or on its behalf a competent authority in the country of establishment, the following is requested:

-samples, description and/or photos of products that are the subject of delivery, the reliability of which the economic operator is obliged to confirm if requested by the contracting authority:

Минимално ниво(а) на стандарди:

- Statement of provided professional technical personnel in charge of installing and servicing the equipment and training of the personnel who will work with the equipment and an attached Certificate for the individuals, issued by the manufacturer of the equipment;

-Decision that the medical device is registered in the Register of Medical Devices in the Republic of North Macedonia, issued by the Agency for Medicines and Medical Devices MALMED or an import permit issued by MALMED

- Statement that it will provide a guarantee of the equipment of at least 2 (two) years, counted from the day of delivery of the item of purchase, during which period it is obliged to perform free service and free replacement of spare parts for the equipment; Statement that it will provide servicing of the equipment of at least 15 (fifteen) years, counted from the day of delivery of the item of purchase

- Statement confirming that the equipment, subject of purchase, will be new and unused

-Technical books or catalogs (or extracts thereof), issued by the manufacturer of the offered products goods, which contain data confirming the required technical characteristics specified in the technical specifications. In them, the technical characteristics of the offered goods should be clearly and unambiguously marked and numbered in accordance with the numbering from the technical specifications. Such prepared (marked and numbered) catalogues, scanned, are delivered exclusively through ESJN. (If the commercially available catalog does not contain all the minimum technical characteristics required in the technical specifications, they must be confirmed by a statement from the manufacturer of the offered goods. The statement from the manufacturer must be submitted in its original form and with a Macedonian translation.)
