The of service from a cleaning and hygiene maintenance service for the needs of the PHI Healthcare Center of Skopje CO Skopje shall be provided in the following facilities: – Polyclinic Bit Pazar, Polyclinic Chair, Polyclinic Bukuresht, Polyclinic Jane Sandanski, Polyclinic Gjorce Petrov, Polyclinic MOI, Polyclinic Saraj, Polyclinic Idadija, Polyclinic Dracevo, Polyclinic M.A. Chento, the Emergency Service, Children and Youth Mental Health Service, Sportpersons Preventive Healthcare Service and Polyclinic Shuto Orizari. The procurement includes daily, weekly, monthly and periodical activities - general cleaning.

Capacity for performing professional activity of economic operator - Document for registered activity: As condition for determining the capacity for performing professional activity, the contracting organ shall require that each economic operator provide a document for registered activity as an evidence that it is registered as natural or legal entity for performing the activity related to the public procurement subject or an evidence that is a member of a relevant professional association in accordance to the regulation of the country where it is registered. The economic operator shall submit the following, as an evidence that it meets the conditions for meeting the conditions for professional capacity: Confirmation for registered activity (DRD Form) as an evidence that it is registered as natural or legal entity for performing the activity related to the public procurement subject or an evidence that is a member of a relevant professional association in accordance to the regulation of the country where it is registered. In the Republic of North Macedonia the competent organ for issuing this document is the Central Registry of the Republic of North Macedonia.

Balance sheet report issued by a competent organ i.e. audited balance sheet report, or abstracts thereof, in cases where the publishing of balance sheet is prescribed by law in the country where the economic operator is registered.: In order to qualify as capable for performing the subject public procurement contract from the aspect of its economic and financial condition, the economic operator should meet the following minimum conditions: - to have realized minimum income, of the same or similar kind as given in the subject to this public procurement, of at least 35.000.000,00 denars, cumulatively, for the following 3 (three) years (2021, 2022 and 2023). If the economic operator has been performing its activity in a period less than the stated 3 (three) years (2021, 2022 and 2023), minimum condition for determining its economic and financial condition is that it has met the required condition in the years for which there are available data. The economic operator shall prove the meeting of the minimum conditions by submitting of the following: -document –balance sheet report and data from the income statement, issued by the Central Register of the Republic of N.Macedonia ((2021, 2022 and 2023).

Technical or professional capacity of the economic operator:: - list of main deliveries of goods or provided services, with values, dates buyers (contracting organs or economic operators), by providing confirmation for deliveries made or services rendered: to have enough experience in providing services for hygiene maintancance in healthcare institutions (contracting organs or economic operators) of which at least 3 (three) successfully realized contracts/framework agreements in the last 3 (three) years as of the public opening of the bids backwards, with minimum square meters of provided services as follows: minimum 1 (one) contracts/framework agreement for 28.000,00 m2, minimum 1 (one) contracts/framework agreement for 23.000,00 m2 and minimum 1 (one) contracts/framework agreement for 20.000,00 m2. The economic operator shall prove the meeting of the minimum conditions by submitting a list of provided services: - reference list in the last 3 (three) years, with value, dates, buyers (contracting organs or economic operators), by providing confirmations for provided services where the areas covered in the provided services shall be stated. -description of the technical personnel and technical bodies that will participate in the performing of the public procurement, especially the ones responsible for the quality control, and in case of public procurement for works, the ones that will be included in the performance of works, regardless of the fact if they are employed in the economic operator or not: to have at disposal with minimum personnel necessary for the realization of the public procurement contract, i.e. minimum 53 (fifty three) persons, of which 45 (forty five) janitors that have valid occupational health certificate and for minimum that have a valid certificate for completed training in occupational health and safety for workers that work on height (practical and theoretical part), minimum 2 (two) persons that have a valid certificate for completed training in occupational health and safety for the position of a supervisor (controller of employees) and for minimum1 (one) person that has a valid certificate for completed training in personal data protection issued by the Agency for Personal Data Protection. The economic operator shall prove the meeting of the minimum conditions by submitting the following: Statement with the description of the technical personnel and technical bodies that will participate in the performing of the public procurement, especially the ones responsible for the quality control by providing the relevant valid documents (occupational health certificate, certificate for completed training in occupational health and safety for workers that work on height (practical and theoretical part), for minimum 2 (two) persons that have a valid certificate for completed training in occupational health and safety for the position of a supervisor (controller of employees) and for minimum1 (one) person that has a valid certificate for completed training in personal data protection issued by the Agency for Personal Data Protection. -statement for the instruments, devices or the technical equipment, that the service provider or the one performing the works has at disposal for performing the contract: to have at disposal or provide the necessary equipment and relevant machines for general and current cleaning of the listed facilities, such as a list of equipment that should at least contain the following devices and equipment: minimum 5 cleaning machines for hard/soft floors –minimum 2 industrial vacuum cleaners – minimum 5 vacuum cleaners with hepa filters- minimum 1 machine for washing under pressure (wap), minimum 2 ladders certified for work on height of up to 9 meters- minimum 4 vehicles for transport of employees and materials – minimum 6 cleaning carts. The economic operator shall prove the meeting of the minimum conditions by submitting the following: a Statement that it has at disposal the necessary equipment and relevant machines for general and current cleaning of the facility with a relevant list, by enclosing invoices and other relevant evidence that it has at disposal the necessary equipment and copies of the traffic license for the vehicles it has at disposal.

Standard: Quality systems standards: In order to qualify as capable for performing the subject public procurement contract from the aspect of the quality system standards, the economic operator should meet the following standards: - The economic operator should have implemented the quality management system standard ISO 9001: 2015 or equivalent. The economic operator should have implemented the occupational safety management standard ISO 45001:2018 or equivalent. The economic operator shall prove the meeting of the standards by submitting the Certificate for quality management system ISO 9001:2015 or equivalent issued by an accredited institution and Certificate for occupational safety management ISO 45001:2018 or equivalent issued by an accredited institution.

Personal condition: In order to prove its personal condition the economic operator is obliged to submit the following documents: - Statement that in the past five years it was not rendered a final court judgment for committed crime pursuant to Article 88, paragraph 1 of the PPL - Confirmation by a competent organ that a bankruptcy procedure was not opened - Confirmation by a competent organ that a liquidation procedure was not opened - Confirmation for paid taxes, contributions and other public duties issued by a competent organ in the country where the economic operator was registered - Certificate from the Registry of penalties for crimes committed by legal persons that minor penalty ban on participation in public tender procedures, awarding contracts and agreements for public-private partnership has not been imposed on him - certificate from the Registry of penalties for crimes committed by legal persons that a minor penalty for temporary or permanent prohibition of performing a specific activity has not been imposed over him; - confirmation that a final judgment - prohibition of professional activity or duty, i.e. temporary prohibition for performing professional activity has not been imposed over him.
