To prove the personal situation, the economic operator should submit the following documents
- a statement that in the last 5 years no final court verdict has been issued for a crime committed by
article 88 paragraph 1 of the Public Employment Service
- confirmation that no bankruptcy proceedings have been opened by a competent authority
- confirmation that no liquidation procedure has been opened by a competent authority
- confirmation of paid taxes, contributions and other public duties from a competent authority of the country where
the economic operator is registered
- confirmation from
confirmation from the Register of punishments for committed criminal acts of legal entities that he has not been sentenced to a secondary one
penalty ban on participation in public call procedures, awarding of contracts for public procurement and contracts for
Public Private Partnership
- confirmation from the Register of Penalties for Criminal Offenses of Legal Entities that no secondary sentence has been imposed on him
penalty, temporary or permanent ban on performing a separate activity
- Certificate that no misdemeanor sanction has been imposed on him by a final judgment - prohibition to perform
profession, activity or duty, i.e. a temporary ban on performing a separate activity

IV.2.3) Economic and financial situation of the economic operator
- Report on the balance sheet issued by a competent authority, i.e. audited balance sheet, or
extracts from the balance sheet report, in cases where the publication of the balance sheet is
prescribed by law in the country where the economic operator is registered: Report on the balance of
status and success certified by a competent authority for the last 3 years (2019, 2020, 2021)
- Statement of the company's total income (data from the income statement issued by a competent authority,
that is, an audited income statement) and, where required, a statement of income in the area covered
with the public procurement contract, and that at most for the last three financial years for which they are available
such information depending on the date on which the company was founded or started operating and in
subject to the availability of such information: Balance Sheet and Income Statement certified by
competent authority for the last 3 years (2019, 2020, 2021)

Technical or professional ability of the economic operator
- with a list of main deliveries of goods or performed services with values, dates, buyers (contractual
authorities or economic operators), by providing confirmation of deliveries or services:
List of services performed in the last 3 years, and at least 3 signed contracts with values,
dates, buyers (contracting authorities or economic operators), by providing confirmation of execution
supplies or servic- with the educational and professional qualifications of the service provider or of the contractor or of
its management staff, provided that they are not evaluated as a criterion for choosing the most favorable offer:
- Statement of technical personnel or engaged technical authorities regardless of whether they directly him
belong to the economic operator and scanned Authorizations B for the performance of at least 2 (two)
Civil engineers issued by the RSM Chamber of Certified Engineers and Architects - Diplomas or
certificates for at least 2 (two) machinists for managing construction machines; - Driving licenses for
at least 3 (one) truck drivers with the appropriate truck driving categoryes performed
- with a statement about the instruments, plants or technical equipment, which the service provider or contractor
of the works is available for the execution of the contract: - Statement that the economic operator
owns or has available: • at least 3 (three) trucks with a capacity of at least 8m³; • at least
1 (one) truck with a maximum capacity of up to 4.5m3; • at least 2 (two) excavators with a weight of at least 16
tons and volume of the basket of at least 1.0 m3; • at least 1 (one) mini excavator weighing up to
maximum 1.8 tons and • at least 1 (one) bulldozer. - Statement that the economic operator has secured
place for depositing the material - the subject of the purchase.
Minimum level(s) of standards possibly required:
- Standards for quality systems: • Scanned valid certificate ISO 9001:2015 or equivalent, •
Scanned valid ISO 45001:2018 certificate or equivalent.
- Environmental management standards: • Scanned valid ISO 14001:2015 certificate
