- statement of the economic operator that in the last 5 years the economic operator or the person who is a member of the management or supervisory body of that economic operator or who has powers to represent or make decisions or supervise him, in the last five years has not a final court verdict for a crime that has elements of the criminal offenses of participation in criminal association, corruption, tax evasion and contributions, terrorism or criminal offenses related to terrorist activities,

oney laundering and terrorist financing and child labor abuse and trafficking, set out in the Criminal Code;

- Certificate for paid taxes, contributions and other public duties from a competent authority of the country where the economic operator is registered;

- confirmation that no bankruptcy procedure has been initiated by a competent authority or a single document proving the ability;

- confirmation that no liquidation procedure has been opened by a competent body or a single document to prove the ability;

- Confirmation from the Registry of Penalties for committed criminal…

… offenses of legal entities that he has not been sentenced to a secondary penalty ban on participation in public call procedures, awarding public procurement contracts and public-private partnership contracts or a single document proving the ability;

… acts of the legal entities that he / she has not been sentenced to a temporary penalty of temporary prohibition to perform a certain activity or a single document for proving the ability;

- confirmation from the Registry of fines for committed criminal acts of the legal entities that he / she has not been sentenced to a secondary penalty, permanent ban on performing a certain activity or a single document for proving the ability;

- confirmation that a misdemeanor sanction has not been pronounced with a final judgment - prohibition to perform a profession, activity or duty or a single document to prove the ability;

- confirmation that he / she has not been pronounced a misdemeanor sanction with a final verdict - temporary prohibition to perform a certain activity or a single document to prove the ability;

Capacity for performing professional activity of the economic operator:

- Document for registered activity -DRD form or certificate for registered activity,

- Decision for performing wholesale of medicinal products in accordance with the Law on Medicines and Medical devices.

Техничка и професионална способност:

a) Statement from the economic operator about the provided expert who will be in charge of installing, maintaining and servicing the equipment, with an attachment Certificate of the expert trained to service the equipment issued by the manufacturer of the equipment. If the expert is not employed, i.e. is hired by the economic operator, the economic operator must also submit proof that he will be available to the economic operator for servicing the offered equipment within and outside the warranty period (agreement with the service provider or with a legal entity where the service provider is employee);

b) Statement from the economic operator, that he will provide service and spare parts after the expiration of the guarantee for at least 5 years from the delivery of the equipment,

c) Statement from the economic operator that he has provided at least 1 expert for training and technical assistance to the personnel of the contracting authority, for handling the equipment.

Минимално ниво(а) на стандарди:

a) The economic operator has provided an expert for installation,maintenance and servicing of the offered equipment, which has a service certificatethe offered equipment for the specific part for which an offer is submitted, issued by the manufacturer of the equipment (for all parts of the procurement item for which he submits an offer);

b) The economic operator should provide spare parts for servicing the offered equipment, at least 5 years from its delivery (for all parts of the procurement item for which he submits an offer);

c) The economic operator should have provided at least 1 expert for training and technical support to the staff of the contracting authority for handling the equipment (for all parts of the procurement object for which he submits

an offer).
