statement that in the last 5 years no effective court verdict has been pronounced for committed criminal act from
Article 88 paragraph 1 of the PPL - certificate that it is not subject to bankruptcy procedure issued by competent
authority - certificate that it is not subject to liquidation procedure issued by competent authority - certificate for paid
taxes, contributions and other public duties by the competent authority in the country where that economic operator is
established
- certificate from the Legal Entities Penal Registry that it is not being imposed a side sanction – prohibition to
participate in public competitions, contract award procedures and PPP award procedures
- certificate from the Legal Entities Penal Registry that it is not being imposed a side sanction - temporary or
permanent prohibition to perform certain professional activity - Certificate that it is not being sanctioned for
misdemeanor with an effective court decision, resulting in prohibition for pursuing professional activity or duty, ie
temporary prohibition for performing professional activity
- Document for registered activity : - Certificate issued by a competent authority in the Republic of Macedonia or
from another country in which the economic operator is registered or a single document to prove the ability in
relation to Article 88 paragraph 2 of the Law on Public Procurement - Copy of the Decision it is determined
that the economic operator fulfills the requirements in terms of space, equipment and staff for carrying out
wholesale trade in medicines and registration in the register of legal entities for wholesale trade in medical
devices, issued by a competent authority in accordance with the Law on Medicines and medical devices
Техничка и професионална способност:
To submit documentation (catalogues, brochures or technical specification) that will prove that the minimum technical characteristics of
the technical specification have been met. In the documentation for proving technical characteristics, those
technical characteristics required in the technical specification of the tender documentation MUST be clearly
marked with a felt-tip marker.
Statement that he will provide a guarantee for the offered item of purchase for at least 1 (one) year, counted
from the day of handover, during which period he is obliged to provide free service and free replacement of
spare parts;
Statement that it will provide spare parts and authorized service in accordance with the legal
regulations for this type of goods in the territory of the Republic of North Macedonia, in a post-warranty
period of at least 5 (five) years
Statement that he will provide expert technical personnel in charge of
installation, maintenance with technical assistance and training of the personnel who will work with the
equipment with a Certificate issued by the manufacturer of the equipment
statement that the goods, the
subject of procurement, for which the offer is submitted, must be new and unused during which period he is
obliged to perform free service and free replacement of spare parts;
