In order to prove its personal status, the economic operator should submit the following documents:

- statement of the economic operator that no effective verdict has been issued against the economic operator within the past 5 years for a committed crime pursuant to Article 88 paragraph 1 of the Law on Public Procurement;

- certificate stating that no bankruptcy procedure has been initiated by a competent body;

- certificate stating that no liquidation procedure has been initiated by a competent body;

- certificate of paid taxes, benefits and other public duties issued by a competent body in the country where the economic operator is registered;

- certificate from the Register of penalties for committed criminal offenses of legal entities proving that no secondary penalty -…

until the receipt of vaccines; - Statement from the economic operator that he will adhere to the principles of Good Distribution practice and the Good Practice for the storage of the veterinary medical agents – vaccine for

oral vaccination of foxes against rabies, i.e. to guarantee that the vaccine for oral vaccination of foxes against rabies will be distributed, stored and transported under strictly controlled conditions; - Statement from the economic operator that the validity date of the vaccines should be from 6 to 8 months when the vaccines are taken over by the contracting authority.
