In order to prove its personal condition the economic operator is obliged to submit the following documents:

- Statement that in the past five years it was not rendered a final court judgment for committed crime pursuant to Article 88, paragraph 1 of the PPL

- Confirmation by a competent organ that a…

… bankruptcy procedure was not opened

… liquidation procedure was not opened

- Confirmation for paid taxes, contributions and other public duties issued by a competent organ in the country where the economic operator was registered

- Certificate from the Registry of penalties for crimes committed by legal persons that minor penalty ban on participation in public tender procedures, awarding contracts and agreements for public-private partnership has not been imposed on him

- certificate from the Registry of penalties for crimes committed by legal persons that a minor penalty for temporary or permanent prohibition of performing a specific activity has not been imposed over him;

- confirmation that a final judgment - prohibition of professional activity or duty, i.e. temporary prohibition for performing professional activity has not been imposed over him.

Capacity for performing professional activity of economic operator:

- Document for registered activity: A condition for determining the capacity for performing professional activity, the contracting organ shall require that each economic operator provide a document for registered activity as an evidence that it is registered as natural or legal entity for performing the activity related to the public procurement subject or an evidence that is a member of a relevant professional association in accordance to the regulation of the country where it is registered. The economic operator shall submit the following, as an evidence that it meets the conditions for meeting the conditions for professional capacity: Confirmation for registered activity (DRD Form) as an evidence that it is registered as natural or legal entity for performing the activity related to the public procurement subject or an evidence that is a member of a relevant professional association in accordance to the regulation of the country where it is registered.

Економска и финансиска состојба:

In order to qualify as capable for performing the subject public procurement contract from the aspect of its economic and financial condition, the economic operator should meet the following minimum conditions: - to have realized minimum income, subject to this public procurement of the same or similar kind, of at least 35.000.000,00 denars, cumulatively, for the following 3 (three) years (2020, 2021 and 2022).

Минимално ниво(а) на стандарди:

The economic operator shall prove the meeting of the minimum conditions by submitting of the following: -document –balance sheet report and data from the income statement, issued by the Central Register of the Republic of N.Macedonia (for the years 2020, 2021 and 2022).

Техничка и професионална способност:

1. To have enough experience in performing services for hygiene maintenance I healthcare institution (contracting organs or economic operators) of which at least3(three) successfully executed framework agreements in the public opening backward, with minimum square footage pf performed services as follows: minimum 1 (one) contract/framework agreement of 20.000 m2, minimum 1 (one) contract/framework agreement of 15.000 m2 and minimum 1 (one) contract/framework agreement of 10.000 m2.

2. To have at disposal minimum necessary personnel for realization of the public procurement contract as follows: Minimum 39 (thirty nine) persons, of whom 33 cleaning persons that have a valid certificate for occupational safety and minimum 5 (five) persons that have a certificate for completed training for occupational safety for workers that work on heights (practical and theoretical part) and minimum 1 (one) person with a valid certificate for participation in training for personal data protection issued by a an agency for personal data protection issued by a an agency for personal data protection.

3. In order to qualify as capable for performing the subject public procurement contract from the aspect of its technical and professional capacity, the economic operator should meet the following minimum criteria: - to have at disposal or provide the necessary equipment and relevant machines for general and current cleaning of facilities, such as a list of equipment that should at least contain the following devices and equipment:

-minimum 5 cleaning machines for hard/soft floors –minimum 2 industrial vacuum cleaners – minimum 5 vacuum cleaners with hepa filters- minimum 1 machine for washing under pressure (wap), minimum 2 ladders certified for work on height of up to 9 meters- minimum 4 vehicles for transport of employees and materials – minimum 6 cleaning carts.

Минимално ниво(а) на стандарди:

1. List of main performed services – a reference list in the last 3 (three) years, with values, dates, buyers (contracting authorities or economic operators), with provision of certificates for performed services, in which the square footage of performed services will be indicated.

2. Statement with a description of the technical staff and technical authorities that will participate in the execution of the public procurement, especially those responsible for quality control with the attachment of valid relevant documents (certificate for safety and health at work, certificate for completed training for safety and health at work for workers who work at height (practical part and theoretical part) and for at least 1 (one) person with a valid certificate for participation in personal data protection training from the Personal Data Protection Agency.

3. The economic operator shall prove the meeting of the minimum conditions by submitting the following: a Statement that it has at disposal the necessary equipment and relevant machines for general and current cleaning of the facility with a relevant list, by enclosing invoices and other relevant evidence that it has at disposal the necessary equipment and copies of the traffic license for the vehicles it has at disposal.

-Quality systems standards: In order to qualify as capable for performing the subject public procurement contract from the aspect of the quality system standards, the economic operator should meet the following standards: - The economic operator should have implemented the quality management system standard ISO 9001: 2015 or equivalent. The economic operator should have implemented the occupational safety management standard ISO 45001:2018 or equivalent. The economic operator shall prove the meeting of the standards by submitting the Certificate for quality management system ISO 9001:2015 or equivalent issued by an accredited institution and Certificate for occupational safety management ISO 45001:2018 or equivalent issued by an accredited institution.
